La Crosse County Health Department

Grant Overview
La Crosse County Health Department-Community Liaison Program will be providing outreach services with a focus on children and Medical Assistance, working cooperatively with local schools, homeless shelters, medical facilities (including the local free clinic), local WIC clinics, and other community partners and resources. The Community Liaison, trained to do health benefits assessments and applications, along with a bilingual Hmong co-worker will focus on the Hmong population within the community. Also, La Crosse County Health Department is working with La Crosse County Human Services in efforts of simplifying the enrollment and renewal process of Medical Assistance. One way of doing this will be by making the Medical Assistance short form available on the internet by going to the La Crosse County Health Department's web site.
