The Public Health Trust (PHT) of Dade County/Jackson Health System

Grant Overview
The Public Health Trust (PHT) of Dade County/Jackson Health System, conducts outreach in South County using workers speaking Spanish, English or Creole to engage in one-to-one enrollment assistance for Florida KidCare. The Coalition also works with many businesses and other community partners to assist them in reaching and helping clients simplifying the health care coverage enrollment process. PHT has engaged in health-related community outreach activities for many years, and has developed an integrated model of outreach and community building. PHT also has a record of raising funds to support its efforts. New and continuing partnerships assure coordinated, sustained efforts.
