Kahuku Hospital

Grant Overview
The Kahuku Local Project is focused on innovative strategies for rural hospital outreach and creating a data tracking system for Kahuku Hospital's social services department and emergency room. One full-time outreach worker and one part-time inreach worker will establish referral systems with schools, health care providers, and community agencies such as WIC, Head Start, Healthy Start, Queen Lili`uokalani Children's Center and Child and Family Services. Their efforts, combined with the data program, can be replicated by other rural hospitals in Hawai`i to improve health insurance enrollment and retention for children and youth.
