Elkhorn Logan Valley Public Health Department

Grant Overview
Elkhorn Logan Valley Public Health Department is based out of Wisner in Northeast Nebraska. ELVPHD will focus on activities to reach all families, with specific activities targeting minorities, primarily Hispanic children and families in Cuming and Madison counties, plus Somalian and Sudanese children and families in Madison county. The project will increase the number of children and families who are able to access health care by conducting an innovative outreach program at Norfolk Community Health Care Clinic and Franciscan Care Services, West Point; simplifying enrollment by contacting families before they access services, and in the billing process; and building on existing outreach about Kids Connection, reaching minority families with information regarding health care coverage programs available to adults and children both in the private realm and through Medicaid/Kids Connection.
